1. Student agrees to attend school regularly, including every Friday and Saturday to
accomplish required services to the public.
2. Student agrees to abide by school's academic policies, rules and regulations. Failure to do
so may result in termination by the school.
3. Student must meet all financial obligation to the school before being permitted to take the
final examination.
4. Students who have finished required services, but lack required hours, may not refuse to perform
any other service until total hours have been fulfilled.
5. If student refuses to perform a service, the refusal must be in written form and signed by the student
and the instructor.
6. Student/Parent or Guardian gives school permission to call upon a physician or refer student to
same for medical assistance in the event of sudden illness or accident. Any expenses incurred
will be the responsibility of the student or his or her parent or guardian.
7. The enrollment agreement is not subject to alteration or cancellation, except by written consent
of the School Director whose decision shall be final and conform to any existing State or
Federal regulations which may apply.
8. School guarantees that all services required by law will be supplied by the institution within the
time limit set forth in this agreement.
9. School assumes no responsibility for negligence, carelessness or lack of skill by
student(s) while practicing on another person during any part of the school course.
10. School reserves the right to terminate any student for non-payment of agreed fees and tuition,
insufficient progress academy or attendance, or failure to comply with rules.
